New Client Forms
To make your first visit even smoother, please complete our New Client Form online. You can also download and print the form below to bring with you to your appointment. This will help us get to know your pet and their health history before we meet!
Parking Information
We understand that parking in downtown Montclair can be a bit tricky, so we’ve made it easy for you with a few simple options:
Metered Parking
There is metered parking available along Bloomfield Ave and surrounding streets, but it can get busy, especially during peak times.
Parking Garage 1: Midtown Parking Deck
Located behind us at 200 Glenridge Ave, this parking garage offers plenty of spaces. Once parked, follow the pedestrian alleyway between 409 Bloomfield Ave (Trend Coffee & Tea House) and 411 Bloomfield Ave (Diamond Cycle). Walk through the alleyway, take a right, and Cameron Animal Hospital will be the fourth building on your right.
Parking Garage 2: South Fullerton Parking Deck
Another option is the garage at 31 S Fullerton Ave. There’s a pedestrian alleyway located between 416 Bloomfield Ave (Lure Lash) and 420 Bloomfield Ave (Red Star Paints). We are directly across the street from the end of this alleyway.
